On Saturday a crane collapsed in New York City resulting in four fatalities and fourteen injuries. Recent reports say that the construction site has been cited 13 times and five violations are still open. An investigation on Friday, the day before the collapse, resulted in no violations.

In Providence on Thursday a worker was killed as a Boston-bound train struck workers inspecting the tracks. Two other workers were injured and the National Transportation Safety board sent a team to investigate what went wrong.
